As our patient, we will be giving you a worksheet with questions to help you In attaining that information.
If you have insurance that provides acupuncture benefits, we will gladly submit your claims for you as a courtesy.
You are responsible for your deductible, co-payment and co-insurance amounts at the time of service.
If your insurance denies payment of a claim or does not remit payment on a properly submitted claim within 60 days, you will be responsible for payment of services.
