To schedule an appointment for an initial consultation, you must speak with Ms. Shurtz or Dr. Grotte by calling (440) 461-7488 from 1pm to 7pm, E.S.T. on Mondays, Wednesdays, and Saturdays.
A package of forms and questionnaires, and an appointment card, will be sent out to you, along with a return envelope.
We must receive them at least one week before your appointment to allow for careful review of the information provided.
Because the history is such an important part of your consultation, we will have to reschedule your appointment if the paperwork is not returned on time, or is incomplete.
