To do this we need a photo of your insurance card front and back, your full name, and date of birth.
We will contact you as soon as we have confirmation of coverage and can then schedule your appointment.
With verified coverage, we will submit claims directly to the insurance company.
Please note, you should expect to cover your co-pay, herbs, and other non-covered services, or deductible owing at the time of service.
